To work as a member of the Hospice team in providing Hospice care within the guidelines of the IDG care plan. Hospice experience preferred.

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Hospice experience preferred.
The Medical Social Worker/Social Work Associate contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement. Hospice experience preferred.
The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive health care services in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. He/She provides comprehensive care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating and collaborating with patients, families and other healthcare practitioners. Hospice experience preferred.
To provide spiritual and emotional support when needed to the Hospice patient/family and to members of the Hospice team. To provide assistance at the time of the patient’s death and support of the family during the bereavement period.
